In order to have your transcript sent to the college to which you are applying, please go to http://cyberguidance.net/college/transcript.htm and follow the directions. Please do this only for the colleges to which you KNOW FOR CERTAIN you are/will apply.

Teacher/Counselor Recommendations:
If you need a teacher recommendation, complete this form:

give the completed form to your teacher, and in order to meet the application deadline, allow AT LEAST 2 weeks from the day you request it.
If you need a counselor recommendation and/or a high school report form completed, see your counselor before you submit the transcript request. Be certain that you have completed an Activities and Experiences Form.
The High School Report Form is a very important part of the application for private colleges. See your counselor to determine if the college requires one.
